What You’ll Do
- Assess patient needs by phone and in-person, using sound clinical judgment.
- Direct patients to the right care—from same-day visits to self-care guidance.
- Collaborate with providers to ensure seamless, safe, and timely care.
- Be a trusted resource for patients, offering not just answers but reassurance.

Requirements
What We’re Looking For
- Current RN or LPN license (State of Vermont in good standing)
- Strong communication and listening skills.
- The ability to think quickly, prioritize, and stay calm under pressure.
- A genuine desire to support patients and their families
- Min 5 years ambulatory outpatient experience
