RN/LPN Triage

Picture this: the phone rings, and on the other end is a patient who isn’t sure if their symptoms mean a trip to the ER, an appointment today, or reassurance that home care is enough. You listen, you assess, you guide—and in that moment, you’re the bridge between worry and clarity, uncertainty and care. At Mountain Community Health, our triage nurses are often the first voice patients hear and the steady hand that helps them take the next right step. We’re looking for a nurse (RN or LPN) who is ready to bring both clinical skill and compassion to this vital role.

What You’ll Do

• Assess patient needs by phone and in-person, using sound clinical judgment.
• Direct patients to the right care—from same-day visits to self-care guidance.
• Collaborate with providers to ensure seamless, safe, and timely care.
• Be a trusted resource for patients, offering not just answers but reassurance.

Requirements
What We’re Looking For

• Current RN or LPN license (State of Vermont in good standing)
• Strong communication and listening skills.
• The ability to think quickly, prioritize, and stay calm under pressure.
• A genuine desire to support patients and their families
• Min 5 years ambulatory outpatient experience
